5k Run/Walk Registration Form

In consideration of this entry, I, the undersigned, intending to be legally bound, hereby, for myself, my heirs, executors, and administrators, waive and release any and all legal rights and claims or damages I may have against the sponsors, coordinating groups, any volunteers, for any and all injuries suffered by me in this event and hold the same harmless hereby. I attest that I am physically fit and am aware of the dangers and precautions that must be taken by me when running/walking in hot and/ or humid weather. I also give permission for the usage of my name and picture in any broadcast, telecast, or print media of this event.
